Award Eligibility

A member of the public who has made a positive impact on the profession of PT in North Carolina (does not need to be an APTA NC member)

• NC General Assembly member, member of Congress, state or federal
• official, respected business person, public figure, or celebrity
• Nomination form (includes Letter of Recommendation) by APTA NC member
• 1 Additional Letter of Recommendation by an APTA NC member
• CV/resume
• Other Letters of Recommendation accepted

Winners of this award are:

• *Demonstrate distinctive support for the PT profession at the state and/or national level.

Winners will receive

• Awards ceremony announcement *Commemorative plaque
• Highlight in APTA NC newsletter & website
• $100 reward to help defray travel and lodging expenses
