Award Eligibility

• Must have at least 7 years of membership in APTA (any Chapter) including 3 years of APTA NC membership
• Must be nominated by a licensed APTA NC member
• Nomination form (includes Letter of Recommendation)
• 1 Additional Letter of Recommendation from an APTA NC member
• CV/resume
• Other Letters of Recommendation accepted

Winners make contributions of exceptional service to the Association

Recipients of this award have outstanding contributions and service of exceptional value to the association.

Recipients must:

• Demonstrate significant contributions and service which may be in any area(s) of the association as a whole, the association's board, districts, SIGs, committees, or delegation. Service may be made through an individual's service on 1 or more elected or appointed position(s) at the chapter and/or national levels.
• Have made persistent and lasting contributions in the following areas: clinical competence, education, community service, and/or

advocacy.

• A nominee's service will be judged based on the results of that individual's service and not merely the duration or continuity of that service 

Winners will receive

• Highlight in APTA NC newsletter & website
• $100 reward to help defray travel and lodging expenses
