Eligibility & Submission Requirements :

• Must have at least 3 years of APTA membership (any Chapter) and at least 1 year of involvement in APTA NC
• Must be nominated by a licensed APTA NC member
• Nomination form (includes Letter of Recommendation)
• 1 Additional Letter of Recommendation by an APTA NC member
• CV/resume
• Other Letters of Recommendation accepted

Award Winners will demonstrate many of the following attributes & contributions:

• *Strong skills and experience in at least one area of PT which includes but is not limited to clinical practice, clinical instruction, management, academics, research, and/or advocacy
• Quality contributions to APTA NC via active involvement

Recipients should have made a difference in the state chapter and profession in any recognizable way, such as:

• *Clinical practice that goes the extra mile, demonstrates a strong commitment to learning that is shared with patients or other providers, promotes legislative issues, recruits new members, enhances PT visibility in the community, enhances research

Winners will receive:

• Awards ceremony announcement *Commemorative plaque 
• Highlight in APTA NC newsletter & website
• $100 reward to help defray travel and lodging expenses
