Please make one combined PDF to upload with the folliowing requirements:
- Relevance to physical therapy
- Title & detailed course description
- Learning objectives *see tips for writing objectives
- Bibliography including five (5) works published within the past ten (10) years
- Presenter/s credentials/qualifications to teach the specific course
- Course agenda/ justification for CEUs
- Copy of course completion certificate
